IMPORTANT: We are bound by certain legal, ethical, and safety requirements for telehealth services. Please assure you meet the criteria below before scheduling your assessment. If you do not meet these criteria, please visit our local provider list for more options or call the Georgia Crisis and Access Line for a referral at 1-800-715-4225.
I certify that I am at least 18 years of age and a Georgia resident.
I certify that I am not currently experiencing a psychiatric crisis including life-threatening suicidality, homocidality, mania, hallucinations, or other symptom of psychosis. I understand that I should not request an assessment if I am experiencing these symptoms. Instead, I should call 911, go to my local Emergency Room, contact a pyschiatric hospital on our provder list, or call the Georgia Crisis And Access Line at 1-800-715-4225 for an appropriate referral.
I certify that I am able to abstain from substance use prior to/during my online evaluation without putting myself at risk of life-threatening withdrawal symptoms.
I understand that I will be charged $150 for this assessment and I must cancel with more than 24 hours notice to avoid being charged.
I understand that I am not scheduling a court mandated "Substance Abuse Evaluation." Kind Recovery does not provide these evaluations which require specific state certification.

Once you request an assessment for a specific time and day on our online calendar, the system will alert us of your request. We will accept your request within 24 hours. Your appointment has not been scheduled until you receive confirmation from us including an invitation to our client portal. All intake paperwork must be completed prior to your scheduled assessment.
